HH provides care management support and community resource connection to at-risk children whose families are struggling to meet the medical, behavioral, and social care needs.

Changing Leads is a program at Hearts and Horses that is dedicated to facilitating discovery, hope, healing, and social and emotional growth for youth at risk through equine facilitated learning.
